Student Health
Our school nurses support student health, safety, and access to learning.
Families may contact their school’s health office with questions about medications, treatments, immunizations, health forms, screenings, or illness-related absences.

Families can download commonly requested health forms. Submit completed forms and supporting documentation directly to the school nurse. Contact the health office if you are unsure which forms are required.
Some forms require completion by a licensed health care provider. Please allow enough time for the provider to complete and return the required documentation before the beginning of the school year.
Health Forms and Documents
Stay up to date on your child’s required immunizations.
Students must meet Pennsylvania’s school immunization requirements or qualify for an exemption or provisional enrollment before attending school. A current copy of each student’s immunization record must be provided to the school nurse.
Pennsylvania permits exemptions for medical reasons, religious beliefs, or a strong moral or ethical conviction similar to a religious belief. Medical exemptions must be documented by a licensed health care practitioner and maintained in the student’s school health record.
Students who do not meet immunization requirements, qualify for an exemption, or have an approved medical plan for completing a required vaccine series may be excluded from school.
Families with questions about a child’s immunization status or access to vaccines should contact the school health office.
Immunization Resources
Submit medication and treatment forms to your school.
Students who need medication or treatments during the school day must have current written orders from a licensed health care provider and signed permission from a parent or guardian. New documentation is required each school year. This requirement also applies to over-the-counter medications that are not covered by the school physician’s standing orders.
To prevent delays:
- Submit medication and treatment orders before the beginning of the school year.
- Provide medication in its original, properly labeled container.
- Have a parent or guardian deliver medication to the school or make arrangements with the school nurse.
- Use the school’s medication form whenever possible.
- Ask the health care provider to complete the appropriate section for Community-Based Instruction, known as CBI, and field trips when flexibility may be medically appropriate.
Medication or treatment cannot be administered until the health office has received all required documentation.

Pennsylvania requires health examinations and screenings at designated grade levels. Depending on the student’s grade and individual needs, school health services may include:
Students receiving special education services complete required screenings and examinations according to the grade level listed in their individualized education program, or IEP.
Families will receive information from the school health office when a screening or additional form is required.
Plan ahead for your child’s individual health needs.
Students with asthma, allergies, seizures, diabetes, or other health conditions may need an individualized action plan and supporting medical orders. Contact the school nurse to determine which forms and documentation are required.

A doctor’s note is required after 10 parent-excused absences.
