21st Century Teacher Survey
Pennsylvania 21st CCLC grantees are required to collect a completed Teacher Survey for each regularly attending student who attends during the school year. A regular attendee is defined as a student who attends 30 or more program days. A math or reading/language arts teacher is recommended. A Teacher Survey is not required for summer-only regular attendees.
Grantees and teachers must use the provided Teacher Survey, as it is aligned to grantee reporting elements. The Teacher Survey should be administered in the spring, as close to the end of the school year program as possible in order to capture a student's growth over the course of the school year.
**Each student's survey must be entered individually. For example, if you have 25 regularly attending students, you need to have 25 completed Teacher Surveys.**
IMPORTANT: You must know the name of the grantee/cohort that a student attends, as students are attributed to programs in a very specific way. Contact the 21st CCLC program if you are unsure which program a student should be attributed to.
