Secondary transition is the process of preparing students for life after they leave high school, including participation in post-secondary education or training, employment and community living. These three areas are often referred to as “post-secondary outcomes” and are the driving force behind Individualized Education Programs (IEPs) written for students in high school.

Transition Discoveries

The Transition Discoveries (TD) initiative works to empower communities to share leadership and collaboration to assist transition age youth, their families, and stakeholders plan for life after high school. TD also offers a framework that helps districts to highlight their strengths while identifying areas of need within nine domains of secondary transition. The domains focus on the following:

This guide is a research-based tool that describes 55 sub-indicators through definitions, characteristics, outcomes, and meaningful stories. The TD guide helps to explain successful transition practices for all areas of preparing students for life after high school.
