SUMMARY:TCCAC Annual Meeting Series: Fostering Success Beyond the IEP
DESCRIPTION:Pennsylvania’s transition community has a long-standing legacy of supporting youth and young adults in successfully navigating the transition from school to adult life. Since 1990\, Pennsylvania’s Local Transition Coordinating Councils (LTCCs) have played a critical role in fostering collaboration\, alignment\, and shared responsibility among stakeholders. \nFor the 2026–2027 school year\, the Transition Coordinating Council of Allegheny County (TCCAC) will host four full-day\, in-person meetings. These sessions are intentionally designed to provide dedicated time for high-quality professional learning\, cross-agency networking\, strategic coordination\, and the development of practical tools and resources. Each meeting will engage transition coordinators\, educators\, administrators\, families\, and agency partners in meaningful learning experiences focused on strengthening collaboration\, enhancing problem-solving\, and improving coordinated supports for transition-age youth with disabilities. \nThe series emphasizes building systems and practices that empower students to successfully navigate their individualized pathways toward graduation\, identify postsecondary outcomes and foster success beyond the IEP. DESCRIPTION:Please join the Allegheny Intermediate Unit’s Training and Consultation team for this HYBRID training series. \n\n\nThrough two in-person training days and two one-hour networking sessions\, this series moves use of the DLM from a PASA assessment score report into a practical tool for instruction and IEP development. Participants will use student specific data alongside the DLM Essential Elements\, Linkage Levels\, and Mini-Maps to identify starting points for instruction\, build learning progressions\, and design meaningful goals\, objectives\, unit and lesson plans aligned to current performance. \nDay 1 focuses on interpreting prior year PASA DLM results into instructional entry points\, connecting Essential Elements across grade levels\, and building unit and lesson plans using high leverage precursor skills.
